Parents and Guardians: below you will find the forms needed to participate in Pendleton Little League for the 2012 season. The Parents Code of Conduct, Player Registration Form, and Medical Release Form must be completed and turned in at the time of registration along with a copy of your child's birth certificate. The cost is $55 per child or $130 for a family of 3 or more. The Volunteer Application is required for all adults who will have contact with kids, such as working in the concession stand, umpires, coaches and managers. This application will include a criminal background check but there is no fee for this.

4. Print, fill out and bring all forms to a registration date.
