Player Forms
If you would like to save time at signups or signup by mail, copy and complete one Medical Release form and three copies of the Player Signup Form for each player being signed up. If you would like a receipt, make a fourth copy for validation. Additional documentation required are an original copy of a birth certificate (copy if signing up by mail), and three proofs of residence. Acceptable documentation for proof of residence are listed on the bottom of the Player Signup Form.

This form replaces all earlier versions. It must be completed by all board members, managers, coaches, and team parents and turned in with a copy of a current driver's license.
