In-House Registration Information:
This section has two files. It is available in Microsoft Word. The first form contains uniform and our clubs refund policy information if your child can no longer play. The second form contains the information for a coach to get his or her childs registration fee refunded at seasons end as the head coach.

Registration Refund Form
If for whatever reason a registration refund is needed from Patriot Soccer Club, please fill out the Registration Refund Form electronically and send to: mjk817@optonline.net. Refunds will be given according to the Club's Refund Policy:
Before August 1, of the registration year : 100% refund
Aug. 1 to Sept. 30, of the registration year: Registration Fee minus $25
After Sept.30, of the registration year: No Refunds
