This form must be completed by anyone planning on being a Manager, Bench Coach or Practice Coach for the upcoming season (available in pdf or word formats). In addition you must also complete the Volunteer form.

2009 PWLL By-Laws

This form shall be completed by all board members, managers, bench and practice coaches and any other volunteer for the upcoming year. You shall submit a copy of a valid picture government issued id along with the completed form.
