We are required to have a W-9 on file for anyone who contracts work from us (typically this will be referees and umpires.)

IRS W-9 FORM

We need the following FOUR forms completed for Payroll employees. This would include field workers and concessions workers.

You can also access the I-9 file with instructions at http://www.uscis.gov/files/form/i-9.pdf

All forms need to be brought to the PYAA office during office hours for verification. We will need a picture id (drivers license or school id) and a Social Security card (copy is okay.) As an alternative, a passport id/card is also accepted for id verification. Paychecks will not be issued until all paperwork is complete.
