This is vital to our manager/coach evaluation process. Information is kept confidential (the forms are reduced to overall statistics and remarks without names, unless you specify otherwise). This process is vital to our selection process next season. Please (a) print the form, fill it out and mail to PRLL, P.O. Box 125, Pearl River, NY 10965 -- mark "EVAL" on envelope; OR (b) place it in a SEALED envelope, with "EVAL" on the envelope, and leave it at the Hopper or give it to a Board member; OR (c) save the form (Word document), fill it out, and e-mail the filled-out form to website@prll.org .
