ASSUMPTION OF RISK, RELEASE AND HOLD HARMLESS 
In consideration of the permission granted by ________________________ (“Owner”) to 
be present on Owner’s property, located at ________________________________________ and 
to observe and/or participate in Oldham County Youth Football League (“OCYFL”) sponsored 
activities including, but not limited to, football and/or cheerleading practice: 
1. The undersigned hereby releases and discharges Owner, its directors, officers, 
trustees, members, employees and agents for all claims, demands, rights of causes of action, 
present or future, whether known, anticipated or unanticipated, and resulting from or arising out 
of or incident to any damage, injury, loss of life, expense or liability of any kind arising out of 
the undersigned’s presence on Owner’s property incident to any OCYFL-sponsored activity. 
2. The undersigned hereby assumes all responsibility for and all risk of damage or 
injury that may occur with respect to his/her presence on Owner’s property for any OCYFLsponsored 
activity. 
3. The undersigned shall at all times indemnify and hold harmless Owner and its 
directors, officers, trustees, members, employees, agents, affiliates and their personal 
representatives, heirs, successors and assigns, and each of them, from and against any and all 
claims, liabilities, losses, demands, penalties, fines, suits, judgments, settlements, damage, costs 
and expenses (including, without limitation, reasonable attorney’s fees) incurred by Owner or 
any other such person or entity, arising out of or related to my, or my child(ren) if a minor, 
presence on Owner’s property. 
I have read and understand and sign the foregoing assumption of risk agreement, release 
and hold harmless for myself and on behalf of my minor children listed below.
