Travel Permit form
Here is the travel permit for team doing exhibition games or tournaments. The coach fills out the form, attaches a team roster and hand delivers a copy to me. It is the visiting team that fills out the form. If a coach invites a team to OMHA ice time it is not a concern of ours. I need 3 working days notice to process these. I will not entertain any late forms. i.e. If the game is on a Sat I need it Tues night at the latest. No Exceptions.
