***Equipment Rental Included in cost***$25 Family Discount (3 or more registrants)*** 

 Registration Requirements 

1.       Player/Cheerleader must be present, along with parent or legal guardian 

2.       Present an ORIGINAL State issued Birth Certificate (and a copy) 

3.       Present a copy of current utility bill (GAS, ELECTRIC or WATER only) that is NOT more than 30 days old    

There will be NO EXCEPTIONS to any of the above
