FC Vipers Registration:
All information is to be completed,(Notarized where indicated) and returned, along with fee and Birth Certificate or Passport. Any Birth Certificate or Passport not in English must be translated, and both translation and (photocopy of)original documents must be submitted.
Football Club Vipers Constitution and Bylaws Forms. Please upload, print, fill out, and return at a scheduled tryout. Fees must be paid prior to entrance to the Club.
