Newfield Soccer Club: Safety & Injuries

Newfield Soccer Club and Long Island Juniors Soccer League (LIJSL) offer a secondary insurance policy for its members.  This secondary insurance is a supplement to each families primary insurance policy.

This secondary insurance policy will cover expenses that exceed the five hundred dollar ($500) deductable.  If your medical expenses exceed this amount, you will be required to notify the club so these bills can be submitted.

However, this process  begins from the day of the injury.  It is imperative that any and all injuries be reported to the Newfield Soccer Club and LIJSL no matter how small you may think the injury.  They must be reported as soon as possible.

To begin, you must complete the Advance Injury Report form and submit it to Newfield Soccer Club and mail to:

You cannot submit a claim form unless you have completed and INJURT REPORT.  This can be found on the LIJSL website.
