- Copy of Birth Certificate
- Wallet size photo
- Copy of final report card of current year

If you are volunteering your time as a NRMF board member, head coach, assistant coach or team mom, below is the Volunteer Form you must complete. In addition, Cheer and Football coaches must complete the Coaching application.

NOTE: Please submit all Coaching Applications during registration. If you prefer to send via email, please scan the signed form and email to Davidt.Garrison@verizon.net for the Football Coaching Application or dor1866@aol.com for the Cheer Coaching Application. If you prefer to send via regular mail, please send the Football Coaching application to Dave Garrison and the Cheer Coaching Application to Doreen Garrison. Address for both is: 15 Brooks Drive, Stony Point, NY 10980. All Applications must be received no later than April 1st.
