WOLFPUPPY CAMP (any age - rising 8th graders) Cost: $125 for all Day Campers Dates: June 12-13 Registration: Friday, June 12, 8am-9am
WOLFPUPPY CAMP - Campers will receive two days of football instruction provided by the NC State Football staff that is tailored to the needs of younger athletes. Camp sessions will begin at 8 am and will run until 1 pm. Registration, drop off, and pickup is at the Wendell H. Murphy Football Center.
HIGH SCHOOL CAMPS: SESSION I (rising 9th - rising 12th graders) Cost: $275 Overnight/$245 Day Camper Dates: June 19-21 Registration: Friday, June 19, 10-noon
SESSION II (rising 9th - rising 12th graders) Cost: $275 Overnight/$245 Day Camper Dates: June 23-25 Registration: Tuesday, June 23, 10-noon
Team camps will provide 3 days of exceptional instruction and team competition supervised by the NC State Football staff. Coaches will instruct individuals within position groups, during game scenarios, as well as in weight room and film room break-out sessions.

VERY IMPORTANT: YOU MUST MAKE A COPY OF YOUR INSURANCE CARD AND INCLUDE THAT WITH YOUR APPLICATION TO BE ADMITTED TO THE CAMP. Your health insurance will be the primary source of coverage if your child is injured or illness occurs. By state law, this cannot raise your insurance premium. Our insurance coverage will assist in covering expenses that your coverage does not (i.e. deductible or co-insurance).
These forms MUST be mailed or faxed to the camp by May 31, 2009!

Medical Form: Each camper is required to have prepared and signed by a licensed care provider the enclosed '2009 Tom O'Brien Football Camp, LLC, Physical Examination/Medical History Form'. The form includes (1) a health history, (2) an immunization record (specific dates required) and (3) a report of a physical exam conducted within the last 24 months. This information is required in order to participate in the Tom O'Brien Football Camp. Please complete the enclosed form and return it by May 31st or you can fax it back to (919) 515-5766.

Authorized Pick-up/Drop-off Form: Due to State Law and camp policy campers will only be released to the individuals who are listed on the provided 'Camper Pick-up Authorization Form'. This applies to all campers. If you are coming with your coach simply list his name. Please complete the enclosed form and return it by May 31st or you can fax it back to (919) 515-5766.

Payment Policy: All tuition and fees must be paid by May 15th. After April 1st, only U.S. cash, cashier's check or money order will be accepted. Failure to comply may result in cancellation. The fee for any check returned by the bank is $25.00.
Refund Policy: DEPOSITS ARE NON-REFUNDABLE. All requests for reimbursement must be done in writing by filling out a “Camper Reimbursement Form” – No reimbursements to any applications cancelled on or after June 1, 2008 due to the camp’s financial commitment to staff, facilities, and dining services being determined by guaranteed camper numbers.
Insurance: If an accident does occur, you will receive the proper medical supervision and treatment - your parent's insurance policy will be responsible for payment. Please note: Neither North Carolina State University, The Tom O'Brien Football Camp, LLC, the camp staff nor anyone connected with the camp will assume any responsibility for accidents, medical or dental, or other expenses incurred as a result of accidents during camp.
Housing: University Towers - No outside food allowed.
Facilities: Practices will be held at practice facilities in and around Carter-Finley Stadium.
Meals: All camp meals will begin with dinner on the first day and end with breakfast on the last day.
Things to bring: Helmet * Football Grass Shoes (preferably molded bottoms) * Sneakers * Pillows, Pillow cases, & Sheets * Athletic Supporters * Sun Screen * Athletic Socks * Sweat Shirts, Tee Shirts, Gym Shorts, & Sweat Pants * Blankets * Toiletries * Laundry Bags * Towels *
Camp Store: The Camp Store will be open at registration, dismissal and in the evenings during camp (merchandise, pizza, etc.)
In case of Emergency, you can be located through the Football Office (919)515-2114 or -2115. Messages left after standard business hours will be checked on a regular basis.
