Please print a schedule, 2008 Team Contact form and FOUR COPIES of pitching affidavit (your preference of format). You must email a list of your coaching staff to ystokes359@aol.com for free entry into the tournament.
PITCHING AFFIDAVIT PROCEDURE: IT IS THE HEAD COACH'S RESPONSIBILITY TO COMPLETE A PITCHING AFFIDAVIT AFTER EACH GAME, HAVE IT SIGNED BY OPPOSING COACH AND SUBMIT SAME TO SITE DIRECTOR. IF A PROTEST IS LODGED AGAINST A TEAM THAT HAS NOT SUBMITTED THE FORM, THEN A FORFEIT MAY OCCUR.
