CORI Forms
MA CORIs Forms, along with a copy of your license must be given to a CORI Committee Member IN PERSON only; it is MA State Law. All volunteers are required to complete a Volunteer Form, CORI Form and provide a copy of your license. Even if you have been CORI'd by the school or some other organization we still have to have a CORI completed.
