Important Forms:
Insurance Binder Form Coaches should keep copies of this information on hand at every game or practice. Ensure that there are at least 2 copies of the claim form (attached as the last page of this document) at each practice and game. Questions about insurance should be directed to Rich Schofield.
CORI Form All coaches, assistant coaches and any volunteers working directly with players are required to complete a criminal offense record investigation (CORI). Forms should be turned in to Dave Verrone and the CORI must be complete before the volunteer participates in practices or at games.
