Please bring your child anytime between 6:00 -9:00 p.m. to have them fitted for their uniform. All players (returning and new) must attend. We do not want to rely on thinking the same size uniform will fit again; we want to make sure the same size will fit. A uniform waiver will be available to take and will need to be completed prior to picking up your child’s uniform. The Head Coaches will be available at the time listed below to answer any questions or concerns you may have.

Submission of all Medical & US Lacrosse Forms must be made by February 26, 2010

• Your child cannot participate in any practices or games until all paperwork is received. All outstanding forms will need to be turned in this night as well. This includes:
  1. A copy of your child’s 2009 physical form
  2. A signed US Lacrosse membership application. Forms will be readily available this night. For those of you who are new to the program, US Lacrosse holds the master insurance for all players.
