Online Registration Instructions

Registration for the 2013 season has began!  If you are registering more than one football player/cheerleader, each child has to be registered separately.  You will have the opportunity to upload the player's birth certificate, equipment form, photo consent form, concussion form and physical form during the registation process.

If you do not upload the birth certificate, equipment form, photo consent form, physical form and/or concussion form, please mail them to:

After 7/19/13, please do not mail any documentation to the P. O. Box, as we will not receive it before the first day of practice.  At that point, you are welcome to fax your documentation to (256) 325-6638, scan and email it to: manypraises@yahoo.com or bring it with you on the first day of practice.

 

Registration Fees

Football: $185     Cheer: $200

 

Football fee includes registration, insurance, a game jersey, game pants, socks, shoulder pads and a helmet.  Fee does not include a mouth piece.  Football players will be fitted and game pants and socks will be ordered at season opening.  The shoulder pads and helmet must be returned at the end of the season.

Cheer fee includes registration, insurance, shell top w/lettering, skirt, Pom Poms, half top, hair bow, bag, briefs, shoes & socks.  Cheerleaders will be fitted and uniforms ordered at season opening.  The uniform and accessories do not have to be returned at the end of the season.

 

PayPal charges $4.65 for football and $5.00 for cheer to utilize this service.  The charge will be automatically added at checkout.  The charge on your credit/debit card statement will read MAA ESP.

Registration fees are non-refundable.



A Complete Registration Packet

For a Registration Packet to be considered complete, you must turn in the following items:

1. 2013 Football & Cheer Application

2. 2013 MAA Equipment Acknowledgement

3. MAA Code of Conduct

4. MAA Physical Form

5. MAA Photo Consent

6. Concussion Form

7. Copy of the player's/cheerleader's Birth Certificate

8. Payment
