2012 Volunteer Form

Coaches, assistant coaches, and anyone in a leadership role or who has player contact must submit a volunteer form as well as a copy of your government issued ID (Driver's licence etc). This form allows the League to conduct background checks of sex offender registries, child abuse and criminal history records. This form must be filled out every calendar year.
