2012 MYBA Spring Registration Form

Official Roster and Player Add/Drop Forms for the MYBA: Roster MUST be completed with all information, ie. Player's Full Name, Address, Date of Birth and School Attended. Additional Each Player MUST have the Parental/Guardian Signature on the ROSTER authorizing the players participation in the MYBA.

Player Add/Form Must be completed , attached Parental Authorization and Copy of Birth Certificate and submitted to the league office for approval prior to the player participating in a game.

MYBA Player Add & Drop Form

The MYBA Parental Authorization/Release Form. This form MUST be completed with all information, signed by the players parent/legal guardian, with a current picture of the player attached, along with a copy of the player's Birth Certificate.

MYBA Parental Authorization Form

A completed Coaches Background Authorization Form is required for each coach listed on the team's Coaches Registration Form. Only those coaches listed and authorized may coach and enter the field and/or dugouts.
