All players must fill out a Player Registration Form and a Player Medical Form. All parents are encouraged to submit a Volunteer Application. Anyone can apply to volunteer.

Persons with nominations to the Board of Directors must fill this form out prior to a general election. Elections are held at the end of the season, and nomination forms must be turned in two weeks (14 days) before closing ceremonies.
