Merritt Island Senior Little League: Forms

Please print, fill in, and bring the Player Application and Medical Release (which is the back side of the application) to Registration. You will need a set for each registered player. Please bring a birth certificate (or copy of one) to the registration as well. Also, bring three (3) proof of residency documents.

If you plan on volunteering as a Manager, Coach, or Board of Director member, or any other position that requires you to be in close proximity with the children, please also fill out and bring the Volunteer Application with a copy of your Driver's License.
