Volunteer Applications for Managers, Coaches and Team Moms. Volunteers must complete BOTH the Little League and MILL Volunteer Applications. A copy of the volunteer's driver's license must be attached to the Little League Volunteer Application.

There are TWO forms required to volunteer. These forms will also be available at Registration:
[1] Little League Volunteer Application 2010(LLVA) required for the mandatory background check. Please complete all sections, and you MUST attach a copy of your driver's license or photo ID to this form.
[2] MILL Application to Manage, Coach, or participate as Team Mom. All sections must be completed.

Special Request Form
This form may be used to request that a child be placed on a team with a specific child and/or coach. The form does NOT apply to all divisions. Completion of form does not necessary guarantee that the league will be able to honor the request.

TEAM / PLAYER Rating Sheets to be completed by Manager
Ratings are to be completed by the Manager/Coaches of the team, and turned into the VP of the Division. Ratings are used during informal and formal drafts by VPs and Managers respectively to distribute the players amongst the teams. Ratings are strictly confidential, and are NOT to be discussed with players or parents.
