2009 Jr. Division Pool Play Schedule

Contains entry letter w/ instructions, pdf. non fillable forms, and other information. Pdf fillable roster forms are available in handouts. Roster forms are also availble in MS Excel format. Signature forms must be signed by all coaches and athletes who are listed on the roster. Parents or legal guardians must sign the form. Form may be scanned and submitted electronically via email. Teams are not entered into the Qualifier Tournament until payment and completed forms are submitted.

2009 MD DQT entry letter with roster

AAU Team Roster form required for entry into the District Qualifier Tournament. This form shall be completely filled out. Signature form must be signed by all coaches, athletes and parents or guardians of athletes listed on the form. Page 1 of fillable form may be completed on line and sent electronically via email, page 2 may be printed signed, then electronically scanned and submitted via email however you must place the team name and age at the top of the form.
