For your registration packet to be considered complete you must have:
1. Completely filled out Participant contract and parental consent form.
2. Rules and Regulations form signed and dated by player and parent.
3. Physical fitness and Medical history form Part I (completely filled out signed and dated by parent/guardian).
4. Physical fitness and Medical history form Part II signed and dated for 2011 by child’s physician.
5. Original Birth Certificate (Pop Warner requirement)
6. Report card for 2010-2011 Scholastic year.
7. Full Payment (checks / money orders made payable to Mattapan Patriots)

The registration team will be available at the following locations
accepting football and cheerleading applications from 6-8pm (unless otherwise noted):

Registration will also take Monday through Friday, from August 1st-August 19th
