MATTAPAN PATRIOTS POP WARNER: Registration Packet

For your registration packet to be considered complete you must have: 

1. Completely filled out Participant contract and parental consent form.

2. Rules and Regulations form signed and dated by player and parent.

3. Physical fitness and Medical history form Part I (completely filled out signed and dated by parent/guardian).

4. Physical fitness and Medical history form Part II signed and dated for 2012 by child’s physician.

5. Original Birth Certificate (Pop Warner requirement)

6. Report card for 2011-2012 Scholastic year.

7. Full Payment (checks / money orders made payable to Mattapan Patriots)

2012 Registrations fees are as follows:

 Register for the 2012 season and save up to $75!! 

****Due to a change in the equipment policy, the registration fees for the 2012 Mattapan Patriots Football & Cheerleading League has been slightly increased to $175, however you can save if you register early.   
  • For everyone who registers at the banquet on 1/29/12 the cost of registration will be $100. However, the payment must be received in full at the banquet and all equipment from the 2011 season must have been returned.
 
  • The cost of registration from 1/30/12 thru 6/30/12 is $125. 
  • Registration will be held on 2/10, 3/9, & 4/13 at the Mattahunt Community Center from 6-8pm.
  • You can also register by contacting Niya Wise at 617 276-6446; Monday thru Friday from 6p til 9p, and Saturdays from 12-5pm. 
    • Registration will also be available at Almont Park once Flag Football begins in April.
    • To secure this rate, the full fee must be received by 6/30/12. 
  • The cost of registration from 7/1/12 thru 7/31/12 is $150.
    • Registration will be held at Almont Park (exact days and times to be determined).
    • To secure this rate, the full fee must be received by 7/31/12.
  • The cost of registration beginning 8/1/12 will be $175. (NO EXCEPTIONS)
