Park & Rec Coach CORI Form

Coaches:
Park & Rec is implementing a new cross-sport CORI check process which will eliminate the need for the current sport-specific CORI checks. The CORI checks will be valid for three years and will be managed by Bari Jarvis, Director of the Parks/Rec Dept.

Please complete the attached form and make a copy of your license (scanned or photocopied) and submit to Bari Jarvis at the Park & Rec Dept at the Community House or send via e-mail to: bjarvis@longmeadow.org.   You will need to have submitted your CORI application prior to coaching your team.
