(Used for teams playing tournaments before the Premier Passcard pick-up meeting date. See the Premier calendar for specific dates. Most Fall teams need to fill this form out and turn it in with their packet. Most Spring teams do not.)

« To go to the list of forms, click on "Handouts" on the left side of this screen. Then click on the form to open it.
« To save a form to your PC, move your mouse over the form and Click. When the "DOWNLOAD" screen appears, Right Click on "Download Now". Then click "Save target as". Then save the form to your pc. If you have any problems opening or saving, please call me. I can email you any file that you have a problem with.

Detailed instructions are listed for Registration in the Managers Meeting powerpoint. PLEASE READ THEM COMPLETELY BEFORE COMPLETING YOUR TRYOUTS. Understanding the registration process and what is required will make your job (and mine) much simpler. If you don't do it correctly, you may end up redoing the paperwork as many times as it takes to get it right.
If you are a new coach, or have a new manager registering the team, I am available to answer any questions. Please call or email with questions. Better to answer a few questions first, than have to redo everything!
