2009 Forms

Coaches: All individuals who wish to coach a football team or cheer squad in the 2009 season must complete this application along with the 2009 volunteer form

Players: All pre-registered participants must attend the official registration to weigh-in (Football only) and be put on the roster of the appropriate team. New players must register at the official registration. Please print these forms and bring them to registration. Do not sign the NWMFL form until you register. The NWMFL Form is also the form you have a doctor sign for the required physical.
