Registration forms are available to print and fill, then can be brought to any LYFL registration/sign-up session. Offered for parents, as a convenience only. This is not an online registration process.
If this is your player's first year with LYFL we will need to verify his/her birthdate by viewing a birth certificate at Registration. We do NOT need to keep a copy of the birth certificate. Please bring this with you, as well as the completed forms.
*One other note: the Release of Liability is 2 pages long, only the first page is necessary.
