LPYAA 2013-2014 HOCKEY REGISTRATION

If you want to sign up to play hockey for LPYAA you need to mail your completed paperwork and payments to:

LPYAA
P.O. Box 129

All players must be registered by October 1st, 2013 with the exception of IP and Mite age players.  At the IP and Mite levels enrollment will be allowed through Dec. 15th, 2013 in an effort to increase the numbers of players.  Extenuating circumstances (such as, but not limited to , a family moving into town, an individual wishing to try playing hockey for the first time, etc.) may present itself in which case the Hockey Committee reserves the right to allow a registration after the above mentioned cut-off dates.  Any player not registered by their respective cut-off date will be charged a $50.00 late registration fee.

Please fill out the attached forms:

1) LPYAA Player Commitment Form – MUST HAVE DATE & TIME SIGNED

2) USA Hockey Participant Code of Conduct (signed by the player)

3) Signature page of the LPYAA Parent Code of Conduct (1 per family)

Mail the above completed forms along with your printed USA Hockey confirmation page(s) to LPYAA, P.O. Box 129, Lake Placid, NY 12946 (you must register each player with USA Hockey before they can be registered with LPYAA).

The detailed instructions and fees are in the attached HockeyRegistration 13-14 document. For first time hockey players (IP) there is no LPYAA fee, just the USA Hockey registration fee.
