This form must be filled out and turned in with registration form. Without this form, player cannot participate.

Bring this along with a copy of the children's birth certificate, 3 proof's of residency and signed copies of the medical release and parent code of conduct forms. Proofs of residency can be valid D.L., auto insurance, auto registration, cable/satellite bill, utility bill (phone, electric and gas only count as 1). Check calendar for sign-up dates.

Manger/Coach Application

If you are interested in Managing/coaching a team this season, a completed coaches application must be turned in for review by the board of directors. Please fill out the attached application and bring to sign-ups or mail to P.O. Box 6186, Kingman 86402.
Applications must be received before Feb. 21, 2010 to be considered.
