This is a template file for MS Word (.dot) You should save it as Type = Template. Then click on File -> New -> select under Templates (at right) "On my Computer", then select the template to create the new document.
These are set up to print on Avery (or similar) 3-1/4" x 4" White Mailing Labels (Avery # 5164, 8164 and related).
