Little League of the Islips: Handouts

Extended Season is to afford children the opportunity to play more baseball/softball during the month of August. We keep no standings nor do we keep score of the games, and there are no playoffs. The intent is to have fun, and to give more instruction to the kids on how to play different positions that they might not have played before during the regular season.

Medical Release Form

In the event of a player injury please follow this procedure within (5) days of the incident in order to be eligible to submit a claim.
1) Download the "Injury Report" off the website.
2) Fill out the second page of the form and sign it
3) Have the Manager fill out the first page of the form and sign it.
4) Hand the completely filled out form in to the Clubhouse.
5) Follow up that it was received. E-mail Sonny Feinman at sfeinman1423@aol.com

If the player has no Primary Insurance or Partial Insurance all bills must be submitted to S.Feinman who will forward all necessary forms and paperwork to Williamsport.
REMEMBER: There is a $50.00 deductible.
