You may print these files, fill them out and either mail them to our office or contact our player agent to make arrangements to meet at our office.

Please remember that the player agent needs to see the players original birth certificate. We also need three proofs of the family's physical residency. Please see the "residency eligibility requirements" file to see what is acceptable to submit.
