Bob Doyle suggested that all parents make a couple of copies of the entire packet.  Keep one in an envelope at home and keep one in an envelope your players bag.  This is to ensure that if any portion is misplaced by health office or coach they have a copy for replacement and to allow them to participate in tryouts and/or games.
