Please fill out this form as well as the Parent Code of Ethics and bring them to walk-in registration. If you want to volunteer as a coach, you are required to complete the coaching application as well. Being placed as a coach on a team also requires successful completion of a background check.

HYAA Fall 2011 Player Application

Parent's Code of Ethics - To be signed by at least one, but preferably both parents. These are codes that HYAA expects ALL parents should follow while attending HYAA functions. Please bring this form to walk-in registration as well.

2011 HYAA Fall Coach Application

For those interested in purchasing billboard space at any of the HYAA fields, please download, complete and either e-mail or send via USPS to our HQ listed on the form. Your billboard will be predominantly displayed year-round on the field of your choosing.

HYAA Team Sponsor Application - Any person/business that would like to sponsor a team needs to complete one of these applications. Once completed, please bring the form to walk-in registration or follow mailing instruction provided on the form. You may also e-mail the completed form to webmaster@hyaabaseball.org.

This form is for people that want to use our facilities, and it allows those facilities to be reserved.

HYAA Facility Use Form

HYAA is pleased to offer group baseball lessons to its players, enabling them to work on their game and to see positive results. Players between the age of 8 and 12 years old who were on a HYAA team roster in either the Spring or Fall season are eligible to participate. For more information click here.
