Insurance Handling For 2005 Season
In order to provide a better service we are changing
how to file Medical Insurance Claims when injury has
occurred participating in Pop Warner Activities.

Effective August 1st, 2005, all injuries that require
medical attention will be reported to Duane.C@comcast.net.
We will need a description of type of injury, location
injury occurred, type of activity along with time and
date. The parent will then contact Joe Dold @ 816-582-4024.
Joe is the agent handling our Insurance. Joe will then
mail you a packet with instructions and forms.
Keep in mind, our Insurance is secondary Insurance. It
is designed to pick up cost that your primary insurance
doesn't pay. Hopefully this will streamline the insurance
process. Let's have a safe 2005 season!
