A copy of the medical release form must be completed and on file before your child is able to play. Please print out the attached handout, fill in the information, and sign and date.

Medical Release Form

A copy of the boundary map indicating where you live with your initials is required to be on file before your child can play. Please review the attached maps below, locate your home on a map, print the map, and initial your location in RED. Please also write your child's name at the top of the map.

We're trying to make it easy for you - open the attached handout, insert your envelope in your printer, and all you need to do is add your return address and a stamp!

If you are volunteering to work with the children in any capacity (helping with practices, team mom, etc.), we are required to have a volunteer application on file for you. Please print out the attached form, complete it, and send it with a copy of your driver's license to Highlands Little League; Attn: Volunteer Application; P.O. Box 2742; Battle Ground, WA 98604. You will not be able to work with the children until this form has been completed and is on file with the League.
