Registration Forms for Regular Season
Please go to "Handouts" on the left of this screen and dowload/print thee documents and bring with you along with your child's three (3) proofs of residency dated between February 1, 2011 and February 1, 2012, AND the ORIGINAL BIRTH CERTIFICATE.
Medical Release Form; Media Release Form; 2012 Volunteer Form (if you are going to volunteer)
Your proof(s) of residency MUST show the physical address where the child/player lives, not a P.O. Box. The President or a Board Member must see the original birth certificate and we will make a copy. This process repeats at All Stars. The original birth certificate is a Little League requirement and has been for years.
