Pop Warner regulations and insurance requirements have specific guidelines addressing completion of player/participant paperwork.   ALL paperwork and fee payments must be complete in order for the player to be able to practice at the first practice. There will be NO EXCEPTIONS this year. Paperwork and fees not received by July 15th may jeopardize your child's position on the roster. Players may be dropped from the roster and will not be entitled to a refund. Please keep in mind that we will not issue uniforms are equipment to anyone with incomplete paperwork or who has not paid in full. Paperwork includes:
(1)   CERTIFIED (official raise seal) Birth Certificate (no photocopies)
(2)   **NEW** Copy of insurance card front & back (required for all JPW, PW, Jr Midget, Midget)

(3) 2008 Physical Fitness & Medical History Form (PWLS National form) - prior to July 15th
(4) 2008 Participant Contract & Parental Consent Form (PWLS National form)
(5) FCB Parent Participation Agreement
