Player Registration & Waiver Form for Tryouts
PRINT OUT, SIGN, AND BRING THIS FORM TO TRYOUTS. NO ONE MAY PARTICIPATE IN TRYOUTS WITHOUT THIS FORM. A PARENT SHOULD ALSO COME TO THE PARENT MEETING.

Individual Player Registration Form - BRING 1ST PRACTICE
This form is to be filled out and returned the first official practice session.

Medical History and Release Form - TURN IN 1ST PRACTICE
This form must be filled out and returned at the first official practice. It must be notarized. Vicki is a notary - and if you wait and sign it before her - she will notarize it.
