Wolfpack 2010 Registration

@ Fishhawk Sports Complex, Multi-purpose field boardroom.
Returning Players and Cheerleaders
from 9am to 12pm
New Players and Cheerleaders
from 1pm to 3pm
Returning players registration is from 9:00 am - noon. These are the families that completed their pre-registration form prior to the end of the 2009 season and paid $50 to reserve their spot on a Wolfpack team. If your name is not on the pre-registered list you will be asked to return and register in the afternoon.
New player registration is from 1:00 - 3:00 pm. This includes all players that did NOT complete the pre-registration as well as any families that are new to the Wolfpack organization.
Registration Forms needed:

2010 Participant Contract and Parental Consent Form

2010 Physical Fitness

Medical History Form
-
Birth certificate
All forms can be found in the "Handouts" tab.
Registration Fees are as follows:

Flag - $125.00

Cheer - $200.00

Football - $200.00
