This form must be successfully completed prior to any adult being assigned as a manager, coach, or umpire for the 2010 season.

Each applicant must also successfully complete an annual background check. Please contact Sue McDonald at 869-4763 for the background check form(s). All adults named as managers and coaches will be issued an I.D. from Frederick County Parks and Recreation and these I.D.'s must be worn and displayed during all practices and games held at county owned facilities.
