Injury Tracking Form

Injury/incident form for completing within 48 hours of time of injury. To be completed by manager/coach who was present when the injury occurred. Use for incidents where a player, volunteer or fan was injured and may have cause to file an insurance claim with ESLL. Submit completed form to the league secretary (cc President, VP and safety officer) within 48 hours of incident. When in doubt, fill one out.
