Eagles Pop Warner Football & Cheerleading: Registration
Welcome to ESMFA Cheerleading for the 2009 Season
1) New photos and copies of birth certificates WILL BE required this year
2) A physical dated after 1/1/2009 must be completed and signed by a licensed physician and on the attached physical form and returned by 7/30/2009 in order for the child to participate. No other physical forms are going to be accepted. NO EXCEPTIONS. This is a Pop Warner regulation.
3) Levels of Jr Pee Wee and higher are required to turn in a copy of last report card prior to July 31st.
4) All games are mandatory- regardless of weather.
a) All levels from mighty mite and above will travel with the appropriate football team. Flag teams are not required to travel- but may do so if coach requires - Please discuss traveling for flag teams with your appropriate flag coach.
5) All participants MUST arrive no less than 30 minutes prior to any game- either be an away game or home game.
6) No jewelry will be worn FOR ANY REASON by participant at any Pop Warner function – either be practice, games and or fundraisers. There will be no exceptions.
7) Uniforms will be supplied for all levels from Mighty-Mite and up will also be required to purchase – a leotard- spankies and cheer sneakers.
8) All uniforms including pom/poms must be returned in a clean respectable state at the last game. Any uniform that is not returned – the parent or guardian will be charged $150.00
9) There will be a meet the coach/ team day Saturday August 1st. on the home field (time to be determined). Actual practices will not start until Monday August 3rd. Discuss actual practice times with corresponding team Coach.

New for 2009 – We have added a fund raising raffle into cost of registration. Included in your fee is 5 - $10.00 raffle tickets (per family) that you may sell and keep the money. You will also receive a # of $1.00 raffle tickets that you may sell and also keep money
