Fundraiser: DUE AT PRACTICE on MAY 9th or Mail to Dana Salvo at the address listed on form.

 

ESPN Magazine Sales Drive
 •         $40 for a 2-year subscription
 •         5 Subscriptions Sold is the Minimum Fundraising Obligation
 •         Earn $75 credit off Club Fee (or rebated if Paid-in Full)
 •         Order Forms with subscription fees due April 23rd
 •         Instruct checks to be made payable to “Dragon Lacrosse Club”
 •         Direct questions and order placements to Dana Salvo Uncapped and
 Accelerated Earnings
 •         Earn $22.50 additional credit for each paid subscription over 5
 Non-Participation Fee (Sell no magazines)
 •         If you choose not to participate, there is a $75 fee
 Partial Participation (Sell less than 5 magazines)
 •         Receive a $15 credit for ea sold, Owe $15 for ea unsold (5 - # sold)
