Coaches' Registration Process
| Thank you for coaching our young players in the league. We are registered through CYSA and all coaches must register to coach. This process includes a background check.
1.) Please download and fill out the following TWICE with wet signatures: http://www.cysanorth.org/Registration/Forms/Fill-In%20Form%201628%20Team%20Official%20Registration%20&%20Risk%20Management%20Disclsoure%20Form%20Rev.%2012-2007.pdf REMEMBER to Make 2 copies with wet signatures. Make sure your SSN or Driver's license number with expiration date is typed on the form. 2.) Turn in a 1" x 1 1/2" photo of your face, no hat or sunglasses, for your pass card (this is only for U8 and above, not the Saturday morning coaches) Thank you!
