In addition to the required forms (listed below), the club will also need:
* A copy of your Semester 1 Report Card
* A copy of your School ID
* A copy of your RMSIHL Registration Form
* A copy of your AAU Insurance Card (Club Code is: XTB78Y and MUST include added benefit/extended coverage). You MUST HAVE AAU INSURANCE TO STEP ONTO THE RINK.
Make sure to bring ALL completed forms/copies with you to our 1st practice (1/3/12). In an effort to be green and conserve paper, we will not be printing extra copies to have on hand. If you are unable to print these on your own, please email the club so we can make arrangements for you.
