  • TOURNAMENTS - Here you will find information related to our tournaments. Registration forms, roster forms, and more
  • OFF SEASON HOCKEY WAIVER - This form must be filled out and turned in before your child attends any of the Crookston off season activities. Including but not limited to: skills, dryland, scrimmages, tournaments...
  • BLC CHECK REQUEST - If you are requesting a check from the Crookston Blue Line Club or to be reimbursed please fill out the Check Request form completely.
  • BLC EXPENSE REIMBURSEMENT - This form would be used if you purchased some items on behalf of the BLC or for the BLC and would like to be reimbursed for that purchase
  • BLC CASH RECEIPT - Use this form if you have any money that needs to be turned into the BLC. Example: T-shirt sales, fundraising money, Advertising money, etc.
  • ICE RENTAL CONTRACT - If you are renting ice from the Crookston Blue Line Club and have already confirmed your dates and times with an authorized Blue Line Club board member, we would need this form completed and returned to Crookston Blue Line Club PO Box 286 Crookston, MN 56716.
  • SUMMER PLAYER INFORMATION - This form should be used to register child for summer hockey activities. Includes, name, birthday, level, parents name, email address.
  • LETS PLAY HOCKEY - If you are interested in submitting information to Let's Play Hockey magazine for your team, this link contains the instructions.

  • GOLF TOURNAMENT - Annual golf tournament fundraiser information.
  • CONCESSION STAND - This will contain concession stand policies and notifications. All parents are required to work throughout the year as part of the parental committment to the youth hockey program.
  • Pirate Hockey Clinic - this is put on each year by Coach Bittner and the boys hockey team
  • Fall League - Information listed here will pertain to the Fall League held in Crookston at the CSC in September.
