McLean County Cougars: Cougar Flag's Locker Room

If you are registering for the Cougars Flag Football 2009 Season, please follow these instructions:
1. Print a copy of each of the five forms linked below.
2. Complete all forms.
3. Make out a check payable to "McLean County Cougars", for the amount of the $25 registration fee.
4. Obtain a copy of your player's birth certificate, or baptismal document, or PA Card, whichever is applicable for you.
5. Mail the five completed forms, a copy of the birth document, and the check payable to the "McLean County Cougars", to:
